Community Facility Linkage- C-it DU-it

Discover how the Community Health Promoters – Case Report Form (CHP-CRF) is transforming Antenatal Care (ANC) at the community level. Developed collaboratively with frontline teams, this tool closes the longstanding gap between household-based antenatal services and linked health facilities. By documenting the essential care provided by CHPs during home visits, the CHP-CRF strengthens follow-up, improves continuity of care, and brings visibility to the critical work CHPs do every day.
 
Thoughtfully integrated into existing systems, the CHP-CRF fits seamlessly into the MOH 216 mother–child booklet, ensuring that every mother’s journey is recorded and connected to facility teams.
